OSHA posts notice of violations at Fono Building site

The federal agency responsible for ensuring the safety of work places, the Occupational Safety and Health Administration has posted a notice inside the new Fono building that’s under construction of violations it found at the site, as well as fines.

The contractor for the project is Paramount Builders.

The notice lists 26 violations dated January 2023 and July 2023. The fines range from $6,629 to $154,696.

The total of all the fines listed in the notice comes to $1.1 million.

The heaviest fine of $154,696 is for no fall protection over 16 ft.

There are five fines of $132,583 for no personal fall arrest systems, no guard rail over 6 ft. fall danger, no guard rails on stairs more than 30 inches, no personal fall arrest systems, and no guard rails on stairs more than 30 inches.

Other violations are open electric panel boxes, working scaffolds not fully planked, no siderails on scaffolding access and employer does not provide fall protection, PPE not provided by employer, amputation hazard for rebar cutoff saw, and no ground fault circuit interrupters.

KHJ News understands that the Fono leadership has been made aware of the OSHA notice which, according to employees, was posted two weeks ago.
